1) Check the Requisiton form that you will receive after you place your order.
2) Go to any Quest Lab location. Your order is sent immediately to the lab. You’ll see the locations you can visit on the next page after placing the order. You can also see a list of locations by clicking here. (Clicking here - link to find a lab page)
3) Visit the lab. Take your Requisition form that will be provided to you after you place your order. Your personal information is never disclosed. Your privacy is safe with us.
3) Get your results! Results will be made available on the Account tab. Logins are available after you place your order.
